Finding a provider or facility is easy! Provider: Gulf Coast Provider Network - for Sarasota and Manatee County. After clicking on the link above, click on Provider Lookup on the left. You can search by a doctor's name, by location, by the type of doctor you need, or any combination. If you are looking for a specific type of physician, enter % and the first 3 letters of the type (i.e. %ped for pediatrician, or %int for internist) in the desired specialty field.Facility: Gulf Coast Provider Network - for Sarasota and Manatee County. After clicking on the link above, click on Facility Lookup on the left. You can search by location or a specific type of facility you need (such as Hospitals, Labs, Occupational Medicine and more). To search for the type of facility click in the type field and view the options.Evolutions Healthcare Systems Network - for those who live out of the local area and have a plan allowing access to the Evolutions network doctors and facilities. Members on the City of Sarasota EPO1 plan who live in Hillsborough, Pinellas or Pasco Counties, may utilize the Evolution network, but only for those three counties. Chiropractic Care - chiropractors in the Evolutions Network are considered in-network.
