Here's How to Receive an Additional
$100.00 on Yur Annual Wellness Benefit for Next Plan Year & Learn More About
Yourself!
- Obtain your current weight, height and waist size along
with your blood pressure and heart rate.
- Access the web site at
http://www.hcare.net/cityofsarasota If you do not have access to a
computer, you can complete the HRA at a computer in Human Resources.
- Click on New Users Click Here at the
bottom center of the page.
- For privacy, pick your User Name and Password
(WRITE IT DOWN - you will need it in the future). Approximately 1 week
after your blood draw you will be able to go back into your HRA where you
may view your lab results.
- ***For confidentiality purposes, Human Resources or
HealthCare Strategies will not have access to any user names
or passwords. You are responsible for their safe keeping.
- Employees and Dependents must enter your First Name and
Last Name into those fields in the HRA as it appears on your medical
insurance card.
- Continue answering the questions until you have
completed the HRA
- You must print the completion page to take to the
Sarasota Memorial Hospital Outpatient Lab at the Waldemere Medical Plaza,
2nd floor - or if you have recently taken the labs below you may enter
them yourself. (You only need to enter the lab values once in a year
period to qualify. Those labs are: total cholesterol, Hdl, Ldl,
Triglycerides and Glucose. (When you take the Completion Page from the
HRA to the SMH Lab, you will not need to pay to take the Lab tests.)
Again, to qualify for the additional
$100.00 on your next plan year's annual wellness benefit you must complete
the HRA, enter your current weight, height, waist size, blood pressure and
heart rate along with the 5 lab values. It does not matter where you obtain
your blood pressure and heart rate reading, but remember there is a blood
pressure station on the second floor in the break-room at City Hall.
At the end of the plan year, if you have satisfied the above qualifications
and you are still on one of the City of Sarasota's Insurance Plans you will
receive $100 additionally to your Annual Wellness Benefit ($400.00 will go
to $500.00). If you have dependents on your insurance and they are
over the age of 18 they are also eligible for this same benefit. There is no
Maximum amount per family.
Create your HRA account:
1. Go to
hcare.net/cityofsarasota
2. In the yellow box, select "New Users Click Here"
3. Fill out the necessary registration information including creating your
exclusive username and password.
4. Complete the Health Risk Assessment and view the results and reports.
5. Follow the directions and proceed to your (PHD) Home page
6. Select the green "My PHD Lesson Plan" to complete your lessons.
Steps for Accessing
Your Account and PHD Lesson Plan
1. Go to hcare.net/cityofsarasota
2. Type in your username and password in the yellow
box.
3. Select the Green "My PHD Lesson Plan" button.
4. Select a lesson from one of the five categories.

The Health Risk Assessment (HRA)

Here’s how to receive up to $200 at the end of the Benefits plan year. Before you take your HRA:
- Obtain your current
weight, height and waist size along with your blood pressure and heart rate
Then:
- Access the web site here:
http://www.hcare.net/sarasota, link is also available on the SMH intranet
(Pulse). If you do not have access to a computer, you can complete the HRA at
a Kiosk in Employee Health on the 5th floor (East Tower) using the
“C” elevator. This program is
strictly confidential and we will not receive individual medical information
about employees or their family members.
- Click on New Users Click

User Name and Password (WRITE IT DOWN – you will need it in the future)
- Employees and Dependents
must enter your First name and Last name into those fields in the HRA as it
appears on your medical insurance card
- Continue answering the
questions until you have completed the HRA
- You must print a
completion page to take to the SMH Outpatient Lab—Waldemere, Blackburn Point,
etc
- The lab will do a simple
blood draw to obtain total cholesterol, Hdl, Ldl, Triglycerides and Glucose
levels (You will not need to pay to take the HRA or the Lab test) (SMHCS
WILL PAY YOU)
At the end of the plan year
if you have completed your HRA, Lab Work, and your current Blood Pressure
Reading and if you are an active employee enrolled in the medical benefits you
will receive $100, and if your dependent is over the age of 18 and completes the
HRA you will be eligible to receive $100 for your dependent. The Maximum amount
per family is $200.
If you have any questions
regarding your HRA, you may contact Healthcare Strategies, the same organization
providing employees and their dependents with the 24/7 Nurse Care Line and
Personal Health Advocate or you can make an appointment with Wendy Hunter, ARNP
in Employee Health.
Reminder: Employees
who complete the HRA are also eligible for a 50% discount on initiation and
membership fees to Healthplex - Sarasota Memorial's medically-oriented fitness
center, located at the Institute for Advanced Medicine, located at 5880 Rand
Blvd.
